Tournament Check-In will be Friday, September 1, from 7:00 PM – 9 PM

at Puryear Park field house. The address for Puryear Park is 5701 Lee Street North,

St Petersburg

The following items are required at Check-In:

1. Official Roster - 4 Copies (Downloaded from the FYSA website, or an FYSA signed by an ADC/DC)
2. Medical Releases - only required at check-in
3. Guest Player Forms – If necessary- 4 copies. (3 guest players maximum allowed)
4. Player Passes

If you are unable to make the Friday night check-in then it must be done an hour before

your first game with the site director. You must bring everything states above if necessary.

If you will be unable to make the check-in then please let me know and we may prepare

to look for you an hour before your game.
